Modafinil and alcohol can both cause dizziness and affect your sleep and judgement, so ask your prescriber before you drink. Modafinil’s bigger interactions are with other medicines, especially hormonal contraception, which it can make less effective.

Key facts

What it is: A wakefulness-promoting medicine.

Licensed use (UK): Excessive sleepiness associated with narcolepsy, with or without cataplexy, in adults.

Legal status: Prescription-only medicine (POM).

Needs a prescription: Yes

Last checked: November 2026

This guide explains what UK sources say about modafinil with alcohol, caffeine and other medicines. It sets out why these interactions happen and which medicines matter most. For an overview of the medicine itself, read our complete guide to modafinil.

Why modafinil interacts with other medicines

Your liver breaks down most medicines using a family of enzymes called cytochrome P450 (CYP). Modafinil changes how some of these enzymes work. This can raise or lower the level of another medicine in your blood.

The UK Summary of Product Characteristics (SmPC) for modafinil describes three main effects:

Effect of modafinil

What it can mean

Examples from the SmPC

Speeds up CYP3A4/5 (induction)

Some medicines are cleared faster and may work less well

Hormonal contraceptives, ciclosporin, HIV protease inhibitors, buspirone, triazolam, midazolam, most calcium channel blockers and statins

May slow CYP2C19 (inhibition)

Some medicines build up and may need a lower dose

Diazepam, propranolol, omeprazole, phenytoin, some antidepressants

May suppress CYP2C9

Some medicines are cleared more slowly

Warfarin, phenytoin

It works the other way too. The SmPC says strong enzyme inducers, such as the epilepsy medicines carbamazepine and phenobarbital, could lower modafinil levels.

Modafinil also stays in your body for a long time. The SmPC gives an effective half-life of about 15 hours after repeated doses. So an interaction can matter even if you take the two things hours apart.

Modafinil and alcohol

What the UK product information says

Neither the SmPC nor the patient information leaflet (PIL) for modafinil lists alcohol as an interaction. They do not give a “safe” amount either. So there is no official UK advice that drinking on modafinil is fine.

The leaflet does ask you to tell your doctor if you have had alcohol or drug problems in the past. The SmPC advises caution in people with a history of alcohol, drug or illicit substance abuse. This is partly because studies have shown modafinil has some potential for dependence.

Why it makes sense to be careful

Some effects of modafinil overlap with the effects of alcohol:

Dizziness and blurred vision. The PIL says these affect up to 1 in 10 people taking modafinil. Alcohol can make you dizzy too.

Sleep. Difficulty sleeping is a common side effect of modafinil. The NHS lists alcohol as a cause of insomnia.

Mood. The SmPC advises caution in people with a history of depression, anxiety or other mental health problems. Alcohol can affect these as well.

Feeling alert on modafinil does not mean alcohol is affecting you less. If you drink, keep within the NHS low-risk guideline of no more than 14 units a week on a regular basis, spread over 3 or more days.

Drinking, modafinil and driving

The SmPC warns that your wakefulness may not return to normal on modafinil. Your prescriber should keep checking how sleepy you are and may advise you not to drive. The PIL says do not drive if you feel dizzy, have blurred vision or still feel very sleepy.

Adding alcohol makes these risks harder to judge. Never drive after drinking, whether or not you take modafinil.

Alcohol and narcolepsy

The NHS narcolepsy treatment page suggests avoiding caffeine, alcohol and smoking before bed. Good night-time sleep is part of managing the condition, alongside medicine. You can read more in our guide to narcolepsy and excessive sleepiness.

Caffeine and modafinil

Caffeine is not listed as an interaction in the SmPC or the PIL. But the SmPC does mention it. It says modafinil is not a replacement for sleep, and good sleep habits may include a review of your caffeine intake.

Caffeine and modafinil can cause some of the same effects. The SmPC lists a fast heartbeat and palpitations as common side effects of modafinil, along with nervousness and sleep problems. Too much caffeine can add to these. Our guide to modafinil, your heart and blood pressure explains why heart checks matter.

How much caffeine is too much?

The European Food Safety Authority (EFSA) review of caffeine gives these figures for healthy adults who are not pregnant:

Up to 400 mg a day, spread through the day, does not raise safety concerns.

Single doses up to 200 mg do not raise safety concerns.

Even 100 mg close to bedtime may affect sleep in some adults.

EFSA estimates a 200 ml cup of filter coffee has about 90 mg and a 250 ml energy drink about 80 mg. These limits are for healthy adults, not specifically for people taking modafinil. Your prescriber may suggest less, especially if you notice palpitations or poor sleep.

Keep track of all sources: coffee, tea, cola and energy drinks.

Avoid caffeine later in the day, so it does not cut into your night-time sleep.

Tell your prescriber if you get a racing heart, chest pain or anxiety.

Hormonal contraception: the most important interaction

The SmPC says modafinil may make steroidal contraceptives less effective, by speeding up CYP3A4/5. The PIL says this includes the contraceptive pill, implants, patches and intrauterine devices that release hormones.

The MHRA Drug Safety Update from November 2020 says you must use effective contraception while you take modafinil and for 2 months after stopping. This matters because modafinil may increase the risk of birth defects. Based on UK sexual health guidance, it advises avoiding these methods with enzyme-inducing medicines such as modafinil:

combined pills, vaginal rings and patches

the progestogen-only pill

the progestogen-only implant

ulipristal acetate emergency contraception

The same update names the copper IUD, the levonorgestrel intrauterine system (hormonal coil) and the progestogen-only injection as suitable long-term methods. For short courses of about 2 months, barrier methods plus your usual contraception may be advised. Our guide to modafinil, pregnancy and contraception covers this in more detail.

Other medicines that interact with modafinil

The leaflet asks you to tell your doctor or pharmacist about every medicine you take. This table summarises the main groups named in the SmPC and PIL.

Medicine or group

What can happen

What usually happens next

Epilepsy medicines: carbamazepine, phenobarbital

May lower modafinil levels

Your prescriber reviews whether modafinil still works

Phenytoin (epilepsy)

Clearance may fall, raising levels

Checks for signs of toxicity; blood levels may be measured

Warfarin

Clearance may fall

Clotting tests regularly for the first 2 months and after dose changes

Ciclosporin (transplant, arthritis or psoriasis)

Levels may fall; one case report showed a 50% drop

Your specialist may monitor levels

Antidepressants: some tricyclics and SSRIs, such as amitriptyline, citalopram or fluoxetine

Levels may rise in some people

A lower antidepressant dose may be needed

Diazepam, propranolol, omeprazole

Clearance may fall

A dose reduction may be needed

HIV protease inhibitors, such as indinavir or ritonavir

Levels may fall

Specialist review

Calcium channel blockers (such as amlodipine or verapamil) and statins (such as atorvastatin or simvastatin)

Levels may fall, so they may work less well

Your prescriber may check your blood pressure or cholesterol

Buspirone, triazolam, midazolam

Levels may fall

Prescriber review

Do not stop or change any of these medicines yourself. Take each one exactly as your prescriber tells you. If a dose needs to change, they will tell you how.

Telling your prescriber and pharmacist

Interactions are easiest to manage when your prescriber knows everything you take. Before you start modafinil, and whenever something changes, mention:

all prescribed medicines, including ones from a hospital specialist

medicines from a pharmacy or shop, such as cold and flu remedies

herbal remedies and supplements

your contraception, including the type of coil or implant

how much alcohol and caffeine you usually have

Only take modafinil that a UK prescriber has prescribed for you. It is licensed only for excessive sleepiness in adults with narcolepsy. The MHRA confirmed it is no longer licensed for shift-work sleep disorder or obstructive sleep apnoea. Both can cause dizziness and affect sleep, and caution is advised if you have had alcohol problems. Ask your prescriber what is right for you.

Can I drink coffee or take caffeine with modafinil?

Caffeine is not a listed interaction. But both can cause a fast heartbeat, nervousness and poor sleep, and the SmPC suggests reviewing your caffeine intake. Avoid caffeine late in the day.

How long does modafinil stay in your system?

The SmPC gives an effective half-life of about 15 hours after repeated doses. That means interactions can still matter many hours after a dose.

Does modafinil stop the pill working?

It may make the pill less effective.
