Other common side effects include nervousness, anxiety, trouble sleeping, dizziness, a fast heartbeat and stomach upset. Rare but serious side effects include skin rashes, changes in mental health and allergic reactions, which mean you should stop taking it and contact a doctor straight away.

Key facts

What it is: A wakefulness-promoting medicine.

Licensed use (UK): Excessive sleepiness associated with narcolepsy, with or without cataplexy, in adults.

Legal status: Prescription-only medicine (POM). It also covers the questions people ask most, such as headaches and anxiety. For an overview of the medicine itself, read our complete guide to modafinil.

How common are modafinil side effects?

The Summary of Product Characteristics (SmPC) for modafinil 100 mg tablets sorts side effects by how often they were reported. Headache is the only one listed as “very common”. The table below groups the main ones in plain English.

How often

Side effects listed in the SmPC

Very common (more than 1 in 10 people)

Headache

Common (up to 1 in 10)

Loss of appetite; nervousness, anxiety, trouble sleeping, low mood, confusion, irritability; dizziness, sleepiness, pins and needles; blurred vision; fast heartbeat or palpitations; flushing; stomach pain, feeling sick, dry mouth, diarrhoea, indigestion, constipation; weakness; chest pain; changes in liver blood tests

Uncommon (up to 1 in 100)

Many effects, including suicidal thoughts, aggression, agitation, abnormal dreams, migraine, tremor, high or low blood pressure, irregular or slow heartbeat, rash, itching, acne, sweating, back and muscle pain, weight gain or loss, raised blood sugar or cholesterol, and hay-fever-like symptoms

Rare (up to 1 in 1,000)

Hallucinations, mania, psychosis

Not known

Severe allergic reaction (anaphylaxis), swelling under the skin (angioedema), hives, delusions, and serious skin reactions such as Stevens-Johnson syndrome, toxic epidermal necrolysis and DRESS

“Not known” means the frequency cannot be worked out from the available data. It does not mean these effects are common. The full list is in the patient information leaflet that comes with your tablets.

Modafinil headache

Headache is the side effect people notice most. The SmPC says it affects about 21% of patients and is usually mild or moderate. It is dose-dependent and usually goes away within a few days.

In a large US trial of modafinil for narcolepsy, published in Neurology in 2000, headache was the most frequent complaint. However, it was not significantly more common with modafinil than with a dummy tablet (placebo).

If you get headaches:

keep taking your dose exactly as your prescriber tells you, and do not change it yourself

ask your pharmacist which painkiller is suitable alongside your other medicines

tell your prescriber if the headaches do not settle after the first few days, or if they are severe

Migraine is listed as an uncommon side effect. If you already get migraines, mention this when your treatment is reviewed.

Modafinil anxiety, nervousness and mood changes

Feeling nervous, anxious or irritable is common with modafinil. Low mood and confusion are also listed as common in the SmPC.

The UK patient leaflet is clear about what to do. It says to stop taking modafinil and tell your doctor straight away if you notice any change in your mental health and wellbeing. Its list of signs includes:

anxiety or nervousness, mood swings or abnormal thinking

aggression or hostility, agitation, or feeling over-excited or hyperactive

depression, or suicidal thoughts or behaviour

psychosis, which can include delusions or sensing things that are not real

feeling detached or numb

The SmPC tells prescribers that if psychiatric symptoms develop, modafinil should be stopped and not restarted. Tell your prescriber before you start if you have ever had depression, anxiety, psychosis or mania. Your specialist can then weigh up whether modafinil is right for you.

Sleep, alertness and driving

The intended effect of modafinil is to reduce excessive daytime sleepiness caused by narcolepsy. It is not a cure, and it may not bring your alertness fully back to normal.

Trouble sleeping

Because modafinil promotes wakefulness, difficulty sleeping (insomnia) is a common side effect. The SmPC asks prescribers to watch for signs of it. If you are struggling to sleep at night, talk to your prescriber rather than changing when or how much you take.

Driving and machinery

The leaflet says modafinil can cause blurred vision or dizziness in up to 1 in 10 people. Do not drive or use machinery if you are affected, or if you still feel very sleepy.

Separately, the GOV.UK guidance on narcolepsy and driving says you must tell the DVLA if you have narcolepsy. You can be fined if you do not report a medical condition that affects your driving, and prosecuted if you have an accident because of it.

Skin rashes and allergic reactions

Serious skin reactions are rare, but they are the main reason the leaflet tells you to watch for any rash. The SmPC says modafinil should be stopped at the first sign of rash and not restarted.

These reactions include Stevens-Johnson syndrome, toxic epidermal necrolysis and DRESS (drug rash with eosinophilia and systemic symptoms). According to the SmPC, serious rashes have appeared 1 to 5 weeks after starting treatment. Isolated cases have been reported after about 3 months.

Stop taking modafinil and contact your doctor straight away if you notice:

a skin rash or itching, especially if it covers your whole body

blistering or peeling skin

ulcers in your mouth, eyes, nose or genitals

a high temperature together with a rash

The SmPC also describes a rare reaction affecting several organs at once (multi-organ hypersensitivity), including at least one death reported after the medicine was marketed. Children have a higher risk of serious rash, which is one reason modafinil is not recommended for anyone under 18.

Heart and blood pressure side effects

A fast heartbeat, palpitations and chest pain are listed as common side effects. High or low blood pressure, an irregular or slow heartbeat and an abnormal heart trace (ECG) are uncommon.

For this reason, the SmPC says modafinil must not be used by people with uncontrolled moderate to severe high blood pressure or heart rhythm problems (arrhythmias). The MHRA’s Drug Safety Update advises that:

a baseline ECG should be done before treatment starts

blood pressure and heart rate should be checked regularly during treatment

modafinil should be stopped if an arrhythmia or moderate to severe high blood pressure develops

Go to your monitoring appointments, and tell your specialist about any heart condition or high blood pressure before you start. Read more in our guide to modafinil and your heart.

Other things to know

Appetite, weight and blood tests

Loss of appetite is common. Weight gain, weight loss and increased appetite are listed as uncommon. Changes in liver blood tests are common, and raised blood sugar or cholesterol are uncommon, so your specialist may arrange blood tests.

Dependence

The SmPC says studies have shown a potential for dependence. It advises caution in people with a history of psychiatric disorders or substance misuse. Take only the dose you are prescribed.

Contraception and pregnancy

Modafinil can make hormonal contraceptives, including the pill, less effective. The SmPC recommends other or additional methods during treatment and for two months after stopping.

In November 2020 the MHRA advised that modafinil should not be used in pregnancy. Early registry data suggested a possible increased risk of birth defects. Talk to your doctor before stopping contraception or if you are planning a pregnancy. Our guide to modafinil and pregnancy explains which contraceptive methods are affected.

Caffeine, alcohol and some other medicines can add to these side effects or change how modafinil works. Our guide to modafinil and alcohol, caffeine and other interactions explains what UK sources say.

Why modafinil is only licensed for narcolepsy

Modafinil used to be licensed for other sleep problems too. In July 2010 the European Medicines Agency recommended restricting its use after a review of psychiatric, skin and heart risks. It concluded the benefits outweighed the risks only in narcolepsy. The UK licence was restricted in the same way.

This is why it is not licensed to improve focus or study performance. Our guide on how to check a UK online pharmacy explains how to spot sellers offering prescription medicines without proper checks.

What to do if you get side effects

Serious signs: for a rash, an allergic reaction or any change in your mental health, stop taking modafinil and contact your doctor straight away, as the leaflet advises.

Bothersome but mild effects: keep taking it and speak to your prescriber or pharmacist. Do not change your dose yourself.

Report it: tell the MHRA about suspected side effects through the Yellow Card scheme.

Narcolepsy can also be managed with regular sleep routines and planned naps, as the NHS explains in its narcolepsy treatment guide. Anyone can report.

Frequently asked questions

How long do modafinil side effects last?

The SmPC says headache usually goes away within a few days. For other side effects there is no set time. Tell your prescriber about anything that lasts or worries you.

Can modafinil cause anxiety?

Yes. Anxiety and nervousness are common side effects. The UK leaflet says to stop taking it and tell your doctor straight away if you notice any change in your mental health.

Is headache a normal side effect of modafinil?

Headache is the most common side effect, reported in about 21% of patients. It is usually mild or moderate and short-lived. See your prescriber if it is severe or does not settle.

Does modafinil cause weight loss?

Loss of appetite is common. Weight loss and weight gain are both listed as uncommon. Tell your specialist if your weight or appetite changes noticeably.

Does modafinil affect the contraceptive pill?

Yes. Modafinil can make hormonal contraception less effective. You need other or additional contraception during treatment and for two months after stopping. Ask your doctor which method suits you.

Related guides

Modafinil: what it is, how it works and who it’s for

Modafinil in the UK: is it legal and is it a controlled drug?

Modafinil and pregnancy: birth defect risk, contraception and breastfeeding

Is modafinil safe?
